Alpha Dental Plan of Colorado

The Alpha Dental Plan is a Discount Network dental plan designed, owned, and administrated by dentists. Since 1990, our voluntary plan has offered the finest professional care available at fees considerably less than you would normally pay. Our plan offers individual dental care at group health care fees.

Details


Limitations and Exclusions

  1. Medical costs associated with dental procedures are not covered
  2. Dentures or appliances will be replaced only after 3 years have elapsed since such dentures or appliances were provided under any Plan program unless the denture or appliance becomes unserviceable due to illness or other causes not controlled by other means Replacement under this plan will be made only if the existing denture or appliance is unserviceable and cannot be made so. Replacement of dentures, appliances or bridgework due to loss or theft is not covered.
  3. Any dental treatment started prior to the Member's eligibility to receive benefits under this plan, or started after a Member's termination, is not covered.
  4. Failure to follow prescribed treatment, or accidents occurring during the course of any treatment, may result in additional charges
  5. Failure to pay a scheduled co-payment at the time service is rendered may prevent future dental services from being received until all fees have been paid in full
  6. Services provided by non-Plan dentists are not covered
  7. Services which, in the opinion of the attending dentist, are not necessary for the patient's dental health or are contrary to dental ethics
  8. Services which are compensable under Worker's Compensation or employer liability laws.
  9. General anesthetics and intravenous sedation
  10. Dispensing of drugs not normally supplied in a dental office
  11. Any dental procedure or service rendered while patient is hospitalized
  12. Myofunctional therapy procedures for training, treating or developing muscles in and around the jaw or mouth, including T.M.J., except by participating specialists.
  13. Any dental procedure or service that cannot be performed in the dental office due to general and/or physical limitations of the member
  14. Expenses incurred for dental procedures initiated prior to member's eligibility with Alpha Dental Plan of Colorado, Inc., or after termination of eligibility If your policy was backdated only those services performed after your money was received by Alpha Dental will be covered
  15. Any services which the Participating General Dentist recommends be performed by a specialist
  16. The liability of Alpha Dental Plan is limited to the return of the actual premium paid for one year's service under plan
  17. Extractions for asymptomatic third molars (wisdom teeth) are not covered Examples of symptomatic include severe decay odontogenic cysts, chronic pericoronitis and infection.
  18. Co-payments listed for metallic restorations do not include the cost of gold or lab fees for codes 2750, 2751 2752, 2790, 2791 and 2792 This charge will be an additional $125.00 (per unit).
